FEMA and the Santa Cruz County Emergency Operation Center have opened a center in the San Lorenzo Valley. The trailer is at Boulder Creek Elementary School, 400 West Lomond Street, Boulder Creek. Assistance will be available from 9 a.m. to 6 p.m through Tuesday. Applicants should be prepared with the following information if available:
- Social Security number
- Insurance policy information
- Address of the damaged primary dwelling
- A description of disaster-caused damage and losses
- Current mailing address
- Current telephone number
- Total household annual income
- Routing and account number of your checking or savings account (for direct transfer to your bank account)

Assistance remains available at the CZU Lightning Complex Recovery Resource Center, open daily from 11 a.m. to 7 p.m. at Kaiser Permanente Arena, 140 Front St., Santa Cruz. No appointment necessary.
