The man killed in an industrial accident at the Monterey Mushrooms facility in Royal Oaks has been identified. Last Thursday, 62-year-old Jose Fierros of Watsonville was found dead in a piece of equipment known as a hopper. OSHA is investigating the incident and working to identify the cause. Records show another employee was killed at the same facility in 2018 and a third employee died at the company’s Texas facility in 2017.
